Insulin prices are high because three companies dominate the market and a complicated web of middlemen makes distributing insulin convoluted and expensive. Pharmacy benefit managers, insurers, and wholesalers all add costs. The result is a system where patients often pay hundreds of dollars for a vial of insulin that costs only a fraction of that to make.
